By completing and submitting this form, the applicant is signing up for a class. Any future student understands that no refund will be given for this class unless cancellation occurs at least 24 hours prior  to the beginning of class. Refund, if applicable, will be processed once verification of payment is received from the bank. Any cancellations after cancellation deadline will receive no refund, but the applicant can transfer to another class within 6 months of the class registration date on this form.
